If you are single -- health insurance is very cheap and good but if you are a family man then it is very expensive. No 401K contribution.
The insurance package provider changed three times when I was there, for example ADP was the first one. You had to pay a portion of health, dental and vision.
There's nothing besides health/dental/vision insurance. The 401(k) plan has no company matching.
There are a couple perks: 1) Unlimited PTO was introduced January 2016. 2) Gym in basement 3) Free coffee Other than that, if it is not legally required then SpineFrontier probably doesn't have it.

They change every 5-6 months. They get worse. They get more expensive. No one manages the plan. No 401k match.
The benefit pack is lacking. It is expensive for the little you receive. The claim to offer flex hours and work from home -- NOT TRUE. There is nothing here but new foosball table.
Overall, most of the package is on par with the industry/region.
Insurance plan is expensive for a family. 401K was lacking and no matching contributions. Generous vacation over time but I found out no one lasts long enough to use it
