Leadership!!! Lack of structure . No one feels the need to improve processes, just start new ones.

Advice to Management

Take employees seriously when they come and tell you major issues with their managers. If multiple people are telling you the same thing, it is probably true. And actually DO something about it. Turnover is going to keep being high if HR and management doesn't take employee reviews into account.
