Poor leadership creating a low moral and poor work culture. Poor planning so much re work, allways starting a project then changing the plans, this happened five times in one day at its worst. Hard rock mining lots of back injuries due to obstacles placed in the way, an example would be not allowing operators to sheet floors and allowing truck drivers to continue , they then blame the truckie and say floor's were ok. must have been 15 back injuries in the short time I was there.
