The leadership team and senior management lacked strategic thinking and direction.
No career progression. Numerous staff have left and direct report to the CEO said "it's a hot market and a project environment and people are going to come and and go." This is their attitude!
No accountability for management (the statement above is an example)
High staff turnover, meaning more work gets dumped on people.
Poor systems necessitating people to use spreadsheets (which actually work better that the more-expensive systems implemented).
Pay has stagnated, essentially market rate.
Frequent restructures which do nothing.
I wouldn't recommend working there to anyone except for a short term role until something better comes along.
