Middle management (project manager) seems to confused about job allocation and achieving efficiency. Rest on laurels too often. Unwilling to adopt new ideas and choose to stick to old methods that are tried and tested. Upper management too focus on running sales and expansion of company and forgets to set the groundwork of startup company right.
