The organization suffers from widespread mismanagement and a lack of clear project direction. There is a prevalent culture of blaming teams for missing requirements rather than actively problem solving. On top of that, micro-management from unqualified individuals frequently hindered progress to finish work. There were also ongoing transparency issues and information gatekeeping from management that created an environment of mistrust.
