• Operations staff constantly relied on workarounds instead of fixing systemic issues. • Meetings celebrated minor wins while repeated failures were buried under talk of learning and resilience. • Legitimate problems were cheerfully reframed and then repeated cycle after cycle. • Honest discussions about dysfunction were unwelcome. • Honest feedback was actively discouraged, forcing employees to pretend everything was functioning when it clearly was not.
