Lack of communication from the top down. No Indepth training, more of "here's the material. Go look for it" which you never have much time to do. Half the required training doesnt even have formal curriculum or testing. Safety policies are a mess and way way to complicated and paperwork heavy No actual equipment ownership from operations/production/packaging.
