• Problems I identified were never actually addressed, and I saw the exact same issues come up over and over • When things failed, the pattern was always the same: acknowledge it briefly, then move to the next thing • I brought up recurring concerns multiple times and the response was basically silence, no changes made • Decision would shift direction without any real explanation about what happened before or why we were changing • Mistakes got discussed in meetings and then vanished from conversation entirely within a few days • It was like accountability stopped at saying something went wrong instead of preventing it from happening again
