Politics, politics, and more politics. Many teams have absolutely abysmal leadership which caused employee flight, but those leaders are in turn protected by even less competent upper management. This has led to massive "brain drain" in the company in all but a few departments which do have good, if indirect, leadership.
Severe misunderstanding of what it takes to ship a successful car from all of upper management. Very poor internal communications from the top down. Many in upper management didn't even know what my group did because they never paid attention in any of the many meetings where we had to report the status of our projects to them.
Toilets that don't flush. Seriously, this is embarrassing to even have to mention here, but the plumbing in the San Jose office is terrible. Good luck finding a bathroom stall that functions post-lunch.
In summary: a very typical ex-Cisco CEO running yet another startup into the ground.
