In person, given sheets of paper with algorithm, data structure, and SQL questions.
Not able to use any references and had to hand write code on sheets of paper.
No real time limit, but took 2 or 3 hours to get through every problem.

The application is working fine, but the response time is slow. On one page of paper write down all the things you would look at to trouble shoot and improve responsiveness.
