FULLSTACK ENGINEER What you’ll work on

You’ll help build and run a production system that already serves real patients and real trials:

• A prescreen and booking platform that handles patient outreach, intake, and scheduling
• An AI agent layer (on AWS Bedrock with Claude) for outreach, prescreen conversations, and trial matching
• The voice and SMS layer that talks to patients in real time
• A CTMS core — candidate state, trial matching, audit, and integrations with Monday and our calendar stack
• HIPAA-compliant infrastructure on AWS, end to end

Real ownership from day one. You won’t be the junior on a big team — you’ll be the second engineer on a platform, with the autonomy and responsibility that goes with it.

What we’re looking for

Experience: 4–7 years building production software end-to-end. You’ve shipped, broken things, fixed them at 2am, and learned from it.

Core stack (what we work in):

• Node.js / TypeScript (backend services and APIs)
• React / Next.js (web frontends)
• PostgreSQL (primary data store)
• AWS — ECS/Fargate, S3, Bedrock, Secrets Manager, CloudWatch

Strong pluses:

• Comfort with LLMs and AI tooling (Claude, OpenAI, Bedrock — building with them, not just calling them)
• Voice / telephony or real-time systems experience (Vapi, Twilio, Telnyx, or similar)
• Healthcare or regulated-industry experience (HIPAA, SOC 2, audit trails)
• Worked solo or in very small teams before — you know what it’s like to own a thing end to end

What we care more about than checkboxes:

• You can pick up a new framework or language when the job needs it
• You write code that another engineer can read and extend in six months
• You think about running the system, not just shipping features — observability, on-call, incident response
• You ask the second question before writing code (what could break? who depends on this?)

We’re flexible on the exact stack experience if the fundamentals and instincts are there.

How we work

• Remote, India hours, with overlap into the early evening to sync with the San Jose team
• Small team, fast decisions — no committees, no theater, your work ships
• AI tooling is part of the workflow — we use Claude and similar tools daily for code, tests, documentation, and analysis. Comfort with AI-assisted development is expected, not a side note
• Diff review and explicit approval before commits. We move fast but we don’t ship sloppy
• Quality over quantity — fewer features done properly beats a backlog of half-built ones

Pay: ₹378,444.32 - ₹1,029,381.08 per year
